Three things that qualify you
If all three describe you, you can almost certainly enroll — and we confirm your coverage before you do.
You have Original Medicare
You're enrolled in Original Medicare — Part A and Part B (also called Traditional or Fee-for-Service Medicare).
You have a covered condition
You're living with high blood pressure, diabetes or prediabetes, chronic musculoskeletal pain, or depression.
You live in the United States
IntelaCare serves Medicare beneficiaries nationwide — care comes to you at home, virtually.
The ACCESS program is for people with Original (Traditional) Medicare. If you're on a Medicare Advantage (Part C) plan, you're not currently eligible — but reach out and we'll let you know your options.
Care for the conditions you live with
The program supports four chronic conditions — and we can help even if you're managing more than one.

Questions people ask us most
How much does this cost me?
Nothing. For eligible people with Original Medicare, the ACCESS program is covered — there are no copays and no out-of-pocket costs. We verify your coverage before you enroll.
Do I need to buy special equipment?
No. IntelaCare works with the devices you already have. If you check your blood pressure or blood sugar at home, you can simply share a photo or screenshot of the reading — there's nothing new to buy or set up.
Will this replace my own doctor?
No. We work alongside your existing doctors, not instead of them. We share regular progress updates with your primary care provider so your whole care team stays informed and coordinated.
What if I have a Medicare Advantage plan?
The ACCESS program is for people with Original (Traditional) Medicare — Part A and Part B. If you're enrolled in a Medicare Advantage (Part C) plan, you're not currently eligible. Contact us anyway and we'll let you know your options.
