This Privacy Policy explains how IntelaCare Longevity, a service of IntelaCare PBC, collects, uses, and shares information about you when you use our website and telehealth services.

IntelaCare Longevity is a service of IntelaCare PBC (“IntelaCare,” “we,” “us,” or “our”). This Privacy Policy applies to the IntelaCare Longevity website, applications, and related telehealth services (collectively, the “Services”). Some of the health information we handle is also governed by the Health Insurance Portability and Accountability Act (HIPAA) and by our Notice of Privacy Practices, as described in the HIPAA and Your Health Information section below.
2. Information We Collect
Depending on how you use the Services, we may collect:
- Account and identity information, such as your name, date of birth, mailing address, ZIP code, phone number, email address, and Medicare or insurance identifiers used to verify your eligibility.
- Health information, such as the conditions you report, readings and photos you share, symptoms, medications, and information from your care. Most of this information is Protected Health Information (PHI) and is protected under HIPAA.
- Payment information, such as coverage details and, if applicable, billing information needed to process payments. Card processing is handled by our payment service providers.
- Communications, including messages, call notes, and SMS text messages you exchange with us, and your related preferences and consents.
- Device and usage data, such as IP address, device and browser type, pages viewed, and similar technical information collected automatically to operate and secure the Services.
3. How We Use Your Information
We use the information we collect to:
- Provide, coordinate, and improve your care;
- Send you reminders, including appointment reminders and prescription order updates;
- Verify your identity and account, including sending account verification codes;
- Verify coverage and process payments;
- Maintain the security of the Services and detect, prevent, and respond to fraud or abuse;
- Comply with our legal, regulatory, and professional obligations.
5. SMS and Mobile Information
If you opt in, we may send you SMS text messages such as appointment reminders, prescription order updates, account verification codes, and support responses. Message frequency varies, and message and data rates may apply. You can reply STOP to cancel or HELP for help at any time.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.
Your SMS opt-in consent is not shared with third parties. We use your mobile number and messaging data only to provide the Services and the messages described above, and we share it only with the service providers that help us deliver those messages, acting on our behalf under the protections described in this Policy.
6. HIPAA and Your Health Information
Much of the health information we collect is Protected Health Information (PHI) under HIPAA. We are required by law to maintain the privacy of PHI, to provide you with notice of our legal duties and privacy practices regarding PHI, and to notify you following a breach of unsecured PHI. Our Notice of Privacy Practices describes how we may use and disclose your PHI and the rights you have with respect to that information.

We maintain administrative, technical, and physical safeguards designed to protect your information against unauthorized access, use, or disclosure. No method of transmission or storage is completely secure, so we cannot guarantee absolute security, but we work to protect your information consistent with applicable law, including HIPAA.

You may request access to, or correction of, your personal information, and you may withdraw SMS consent at any time by replying STOP. Depending on where you live, you may have additional rights under applicable state law. Rights regarding your PHI are described in our Notice of Privacy Practices. We retain your information for as long as needed to provide the Services, to maintain required medical and business records, and to comply with our legal obligations. Retention periods for medical records are set by applicable law.
10. Children's Privacy
The Services are intended for adults 18 years of age and older. We do not knowingly collect personal information from children. If you believe a child has provided us information, please contact us so we can take appropriate action.
11. Changes to This Policy
We may update this Privacy Policy from time to time. When we do, we will revise the effective date shown above. Your continued use of the Services after changes become effective means you accept the updated Policy.
